Client Intake Questionnaire

The form every new traveler fills out before you plan a thing.

This is yours to deploy. Copy it into your tool (Tern, a Google Form, JotForm) and send it to every new lead. The first three fields are the most valuable: they capture the name, email, and phone straight into your Manifest. The rest gives you everything you need to plan, before you ever get on a call.
